Coordinated Family Support (CFS) is a holistic, person-centered service designed to help adults with developmental disabilities live safely and successfully in their family home. CFS supports the entire household by coordinating care, services, appointments, training, and community resources.

What This Service Is

CFS is a comprehensive support service for adults who live with their families and need help navigating multiple programs, providers, and daily care needs. The goal is to reduce stress on families, increase independence for the individual, and ensure long-term stability in the home.

This service helps families access resources, understand Regional Center supports, coordinate medical and therapeutic appointments, and manage behavioral, health, and daily living needs.

How the Service Works

Step 1 – Family Assessment: We learn about the individual’s needs, routines, culture, strengths, and barriers.

Step 2 – Plan Development: We create a personalized support plan for the household.

Step 3 – Coordination: We help manage services, appointments, providers, and schedules.

Step 4 – Training & Support: We equip families with tools to improve safety, communication, and daily living.

Step 5 – Ongoing Monitoring: We stay involved, update plans, and ensure supports remain effective.

Our team understands the IRC and SLARC systems and works closely with Service Coordinators to ensure families receive seamless support.

How to Request This Service

Coordinated Family Support (CFS) is a Regional Center–funded service. Families must request it through their Inland Regional Center or South Los Angeles Regional Center Service Coordinator.

Steps to Request CFS:

• Contact your IRC/SLARC Service Coordinator by phone or email.

• State that you are requesting Coordinated Family Support (CFS).

• Ask that your referral be sent to Community Evolution Acquisition as your preferred provider.

• Your Service Coordinator will review the request and notify you once it is approved.
