Service Coordination and Case Management focuses on ensuring that individuals receive the support and resources they need to live fulfilling and independent lives. Our coordinators work closely with each individual to assess their unique needs, strengths, goals, and preferences. Through this process, we develop a personalized Individualized Service Plan (ISP) designed to support personal growth, independence, and overall well-being.
Our team works collaboratively with healthcare providers, therapists, family members, and community organizations to coordinate services effectively. Independent Living Skills Support helps individuals develop the everyday abilities needed to live safely and confidently in their own homes and communities. Our staff also help individuals build routines that promote personal hygiene, grooming, and time management. By strengthening these life skills, we empower individuals to increase their independence, improve their quality of life, and build confidence in managing their daily responsibilities.

Health and Medical Coordination ensures individuals receive the proper medical care and health support needed to maintain a healthy lifestyle. Our team assists with scheduling doctor visits, therapy appointments, and other essential healthcare services while helping individuals stay organized and informed about their medical needs.
We also provide medication reminders, support communication between individuals and healthcare providers, and encourage healthy habits such as proper nutrition, physical activity, and wellness routines. By monitoring health and safety concerns, we help ensure individuals receive consistent and reliable healthcare support.
